Metallic Stent Placement In Malignant Esophageal Stricture
Metallic stent placement for malignant esophageal stricture is an important palliative interventional endoscopic procedure performed at Moolchand Hospital, one of the best gastroenterology and cancer hospitals in Delhi. Esophageal cancer — predominantly squamous cell carcinoma and adenocarcinoma — frequently presents at an advanced, unresectable stage, with progressive dysphagia (difficulty swallowing) being the most distressing symptom that severely impairs nutrition, quality of life, and overall functional status.
A self-expanding metallic stent (SEMS) is a braided or laser-cut mesh tube made from nitinol (nickel-titanium alloy) or stainless steel that is delivered in a compressed form within a catheter and self-expands to its full diameter (typically 18–25mm) once deployed across the stricture. SEMS are available in covered, partially covered, and uncovered configurations — covered stents prevent tumour ingrowth through the mesh but have higher migration rates, while uncovered stents anchor firmly but are prone to tumour ingrowth causing re-obstruction.
The procedure is performed under sedation using a flexible upper gastrointestinal endoscope combined with fluoroscopic guidance. The stricture is assessed endoscopically and the upper and lower limits are marked with fluoroscopic clips or contrast injection. A guidewire is passed through the stricture, and the stent delivery system is advanced over the wire and positioned across the stenosis under real-time fluoroscopic and/or endoscopic visualization. The stent is deployed by retracting the outer sheath, allowing self-expansion across the stricture to restore a patent lumen.
Patients typically experience dramatic improvement in swallowing ability within 24–48 hours of stent deployment. The procedure takes approximately 20–45 minutes and can be performed as a day-care or overnight procedure, with rapid return to oral nutrition.
