Limb Salvage Surgery
Limb salvage surgery represents the standard of care for most primary bone and soft tissue sarcomas of the extremities, performed by specialist orthopaedic oncologists at Moolchand Hospital — one of the best cancer and orthopaedic surgery hospitals in Delhi. It involves the en-bloc resection of the tumour with adequate surgical margins, followed by reconstruction of the skeletal and soft tissue defect to restore a functional, preserved limb — providing oncological outcomes equivalent to amputation in appropriately selected patients.
The most common indications are osteosarcoma, Ewing's sarcoma, chondrosarcoma, and malignant soft tissue sarcomas of the limb. Pre-operative staging with MRI for local extent assessment, CT chest for pulmonary metastases, and bone scan or PET-CT for skip lesions or distant disease is essential. Most patients with high-grade bone sarcomas receive neoadjuvant (pre-operative) chemotherapy to reduce tumour size, treat micrometastatic disease, and facilitate limb-sparing resection. Pathological response to neoadjuvant chemotherapy is an important prognostic factor.
Skeletal reconstruction after bone tumour resection employs several techniques depending on the resected segment, patient age, and tumour location. Options include endoprosthetic reconstruction (custom or modular metallic implants replacing the resected bone and associated joint — the most common technique for adults), osteoarticular allograft (cadaveric donor bone), allograft-prosthesis composite, vascularized or non-vascularized fibular autograft, and rotationplasty (a specialized reconstructive option for selected femoral resections in children).
Soft tissue reconstruction to achieve adequate wound closure and coverage may require local muscle flaps or microvascular free flaps when resection leaves significant soft tissue deficiency. Post-operative adjuvant chemotherapy and/or radiation therapy follows surgery for high-grade sarcomas.
