Hemi Glossectomy
Hemi glossectomy is a major oncological surgical procedure performed by expert head & neck and surgical oncology surgeons at Moolchand Hospital, one of the best cancer hospitals in Delhi. It involves the resection of approximately half of the tongue — either the anterior (front) or posterior (back) portion — typically as treatment for squamous cell carcinoma of the tongue, which is among the most common oral cancers, particularly in patients with a history of tobacco use, alcohol consumption, or HPV infection.
The tongue is a complex, highly functional organ critical for speech, swallowing, chewing, and oral hygiene. The decision to perform a hemi glossectomy is based on tumour size (T2 or T3 tumours), location, depth of invasion, and involvement of adjacent structures. Smaller tumours may be managed by partial glossectomy, while larger tumours involving more than half the tongue may require total or near-total glossectomy.
In most cases of hemi glossectomy, simultaneous neck dissection (removal of cervical lymph nodes) is performed to address regional lymph node metastasis, which is common with tongue cancers due to the rich lymphatic drainage of the oral cavity.
Reconstruction of the tongue defect is a critical component of hemi glossectomy to maximize post-operative speech and swallowing function. Options include primary closure for smaller defects, local flap reconstruction (nasolabial flap, buccal flap), or microvascular free flap reconstruction using the radial forearm free flap or anterolateral thigh free flap for larger defects. The radial forearm free flap is the gold standard for tongue reconstruction due to its thin, pliable tissue that allows good tongue mobility.
