Pain Management
Pain management is a specialized and compassionate service offered at Moolchand Hospital, one of the best hospitals in Delhi with a dedicated pain clinic. Pain — whether acute (resulting from injury, surgery, or infection) or chronic (persisting beyond 3 months, such as in arthritis, cancer, neuropathy, or back pain) — is one of the most prevalent and debilitating medical conditions, significantly impacting physical function, mental health, sleep, work capacity, and overall quality of life.
Effective pain management at Moolchand Hospital begins with a comprehensive pain assessment — including characterization of pain type (nociceptive, neuropathic, visceral, mixed), intensity (VAS/NRS scores), duration, location, aggravating and relieving factors, functional impact, and psychological comorbidities (anxiety, depression, catastrophizing). This multidimensional assessment forms the foundation of an individualized, multimodal treatment plan.
Pharmacological management follows the WHO analgesic ladder — from non-opioid analgesics (paracetamol, NSAIDs, COX-2 inhibitors) for mild pain, through weak opioids (tramadol, codeine) for moderate pain, to strong opioids (morphine, oxycodone, fentanyl, buprenorphine) for severe pain. Adjuvant medications — including anticonvulsants (gabapentin, pregabalin for neuropathic pain), tricyclic antidepressants (amitriptyline), SNRIs (duloxetine for fibromyalgia and diabetic neuropathy), and topical agents (lidocaine patches, capsaicin cream) — address specific pain mechanisms.
Interventional pain procedures include epidural steroid injections and nerve blocks (cervical, thoracic, lumbar) for radiculopathy and spinal pain, facet joint injections and medial branch blocks with radiofrequency ablation for facet arthropathy, trigger point injections for myofascial pain, sympathetic nerve blocks for CRPS and visceral pain, intrathecal drug delivery systems for refractory cancer pain, and spinal cord stimulation (SCS) for neuropathic pain unresponsive to medication.
Physiotherapy, TENS, heat, ultrasound therapy, and cognitive-behavioral therapy (CBT) for pain address the physical and psychological dimensions of chronic pain. Palliative pain management for cancer patients combines opioids, nerve blocks, and adjuvants to achieve maximum comfort.
