Laryngeal Mask Airway (LMA) Insertion
Laryngeal Mask Airway (LMA) insertion is a fundamental airway management technique used extensively by anaesthesiologists and emergency physicians at Moolchand Hospital, one of the best surgery hospitals in Delhi. The LMA is a supraglottic airway device — an elliptical mask mounted on a flexible tube — that is inserted blindly through the mouth into the pharynx and positioned over the laryngeal inlet (the entrance to the trachea). When correctly placed, the inflated cuff seals around the laryngeal opening, allowing ventilation and anaesthetic gas delivery without the need for endotracheal intubation.
The LMA was introduced in the 1980s and has revolutionized anaesthetic airway management. It occupies an intermediate position between a face mask (which provides no airway protection) and an endotracheal tube (which provides definitive airway control but requires direct laryngoscopy and carries more trauma risk). The LMA is particularly suited for elective surgery of short to moderate duration in patients who are not at risk of aspiration, including procedures involving the limbs, eyes, ears, and body surface.
LMA insertion is considerably simpler and faster than endotracheal intubation, requires less depth of anaesthesia, avoids the need for muscle relaxants in most cases, and is associated with less cardiovascular stimulation, reduced postoperative sore throat, and smoother emergence from anaesthesia. It is a critical rescue device when endotracheal intubation fails or is difficult — as recommended by difficult airway management algorithms.
Modern LMA designs include the classic LMA, ProSeal LMA (with a gastric drainage channel reducing aspiration risk), Supreme LMA, Fastrach (Intubating LMA allowing blind or fibreoptic endotracheal intubation through the device), and the i-gel (a gel-cuffed single-use supraglottic airway). The choice of device is tailored to the patient, surgical procedure, and clinical context.
