Fractional Flow Reserve (FFR)
Fractional Flow Reserve (FFR) is a gold-standard invasive physiological measurement used in interventional cardiology at Moolchand Hospital, the best heart hospital in Delhi. FFR is performed during cardiac catheterization to precisely assess whether a narrowing (stenosis) in a coronary artery is actually restricting blood flow to the heart muscle significantly enough to cause ischemia (reduced oxygen supply) and warrant revascularization through stenting (PCI) or bypass surgery (CABG).
While coronary angiography provides an anatomical picture of the coronary arteries showing the degree of stenosis, it does not directly measure the functional impact of a blockage on blood flow. FFR provides this critical hemodynamic information, allowing cardiologists to make more informed, evidence-based decisions about which lesions truly need treatment and which can be safely managed medically.
The procedure involves threading a pressure-sensing guidewire across the coronary stenosis during cardiac catheterization. A drug called adenosine is administered intravenously or intracoronarily to induce maximum (hyperemic) blood flow. FFR is calculated as the ratio of the pressure distal to the stenosis divided by the aortic pressure proximal to the stenosis during maximum flow. An FFR value of 0.80 or less indicates hemodynamically significant stenosis that is likely causing ischemia and warrants revascularization. Values above 0.80 suggest the stenosis can be safely treated medically.
Multiple landmark clinical trials (FAME, FAME 2) have demonstrated that FFR-guided PCI significantly reduces unnecessary stenting, improves patient outcomes, and reduces healthcare costs compared to angiography-guided treatment alone.
