Mesenteric Caval Anastomosis
Mesenteric caval anastomosis is a specialized vascular and hepatobiliary surgical procedure performed at Moolchand Hospital, one of the best surgery and liver hospitals in Delhi. It is a form of portosystemic shunt surgery — a procedure that creates a direct connection between the high-pressure portal venous system and the low-pressure systemic venous system (inferior vena cava) to decompress portal hypertension.
Portal hypertension — elevated pressure in the portal venous system supplying the liver — is a serious complication of cirrhosis, portal vein thrombosis, hepatic fibrosis, and other liver diseases. It causes esophageal and gastric varices (dilated, fragile veins prone to catastrophic bleeding), ascites (fluid accumulation in the abdomen), splenomegaly (enlarged spleen with hypersplenism), and hepatic encephalopathy.
In mesenteric caval anastomosis, a surgical connection is created between the superior mesenteric vein (SMV) — draining blood from the small intestine — and the inferior vena cava (IVC) using a prosthetic graft (typically H-shaped or C-shaped using PTFE or Dacron) or direct anastomosis, effectively diverting portal blood flow away from the obstructed liver circulation and decompressing the portal system.
This procedure is particularly useful when the portal vein itself is thrombosed (making direct portocaval anastomosis impossible) or in the mesocaval shunt configuration. It is indicated for prevention of recurrent variceal hemorrhage resistant to endoscopic treatment, refractory ascites, and as a bridge to liver transplantation. Modern endovascular alternatives such as transjugular intrahepatic portosystemic shunt (TIPS) have largely replaced surgical shunts in many centers, but surgical mesenteric caval anastomosis remains valuable in selected patients where TIPS is technically not feasible or has failed.
