Intravascular Ultrasound
Intravascular Ultrasound (IVUS) is a sophisticated invasive imaging modality used in interventional cardiology and vascular surgery at Moolchand Hospital, the best heart hospital in Delhi. Unlike conventional coronary angiography — which provides a two-dimensional silhouette of the vessel lumen — IVUS uses a miniature ultrasound transducer mounted at the tip of a flexible catheter to generate high-resolution, real-time, cross-sectional (360-degree) images of the vessel wall and lumen from within the artery itself.
IVUS provides unique information that angiography alone cannot deliver, including precise measurement of lumen dimensions (diameter and cross-sectional area), plaque burden and composition (distinguishing fibrous, calcific, and lipid-rich plaque), vessel wall thickness, identification of plaque vulnerability, and detection of dissection or thrombus after coronary intervention.
In interventional cardiology, IVUS is used to optimize coronary stent implantation — ensuring adequate stent expansion, complete apposition of stent struts to the vessel wall, and absence of geographic miss or edge dissection. IVUS-guided PCI has been shown in multiple clinical trials to improve outcomes compared to angiography-guided stenting alone, reducing stent thrombosis, restenosis rates, and major adverse cardiac events, particularly for complex lesions including left main coronary artery disease, bifurcation lesions, long diffuse disease, and calcified lesions.
IVUS is also used diagnostically to evaluate intermediate severity stenoses that are anatomically ambiguous on angiography (complementary to FFR physiological assessment), detect cardiac allograft vasculopathy in heart transplant recipients, evaluate spontaneous coronary artery dissection (SCAD), and assess aortic pathology including aortic dissection and aneurysm extent.
