The Ligation of Intersphincteric Fistula Tract (LIFT) procedure is a modern sphincter-preserving surgical technique for the treatment of complex anal fistulas performed at Moolchand Hospital, one of the best hospitals for fistula treatment in Delhi. Anal fistulas are abnormal tunnels connecting the anal canal (usually originating from an infected anal gland) to the skin surface around the anus, causing persistent discharge, pain, recurrent perianal sepsis, and significantly reduced quality of life.
Trans-sphincteric fistulas — which pass through the external anal sphincter — represent the most challenging type of anal fistula to treat because any surgical technique that simply lays open (cuts through) the entire fistula tract would divide a significant portion of the external sphincter, risking faecal incontinence. This dilemma between achieving high cure rates and preserving continence is the central challenge in anal fistula surgery.
The LIFT procedure, first described by Rojanasakul in 2007, elegantly resolves this dilemma by accessing the fistula tract in the intersphincteric space — between the internal and external anal sphincters — through a small curved incision. The fistula tract is identified, carefully dissected free from the surrounding sphincter muscle, securely ligated (tied off) with absorbable sutures on the internal sphincter side, and divided. The intersphincteric wound is closed, and the external opening is curetted and left open to drain. At no point is any sphincter muscle divided, preserving the complete integrity of the anal sphincters and continence mechanism.
LIFT is suitable for trans-sphincteric fistulas that have been drained with a seton suture (a loose stitch placed through the fistula tract prior to LIFT to reduce inflammation) and for selected high trans-sphincteric fistulas in patients for whom fistulotomy would risk continence impairment. Success rates range from 60–80% and can be improved by combining LIFT with bioabsorbable mesh reinforcement (LIFT-PLUG technique) or collagen plug placement.
