Hiatus Hernia Repair
Hiatus hernia repair is a specialized gastrointestinal surgical procedure performed at Moolchand Hospital, one of the best gastroenterology and surgery hospitals in Delhi. A hiatal hernia occurs when part of the stomach protrudes upward through the oesophageal hiatus — the opening in the diaphragm — into the chest cavity. There are two main types: sliding hiatal hernia (most common, where the gastroesophageal junction slides into the chest) and paraesophageal or rolling hernia (where part of the stomach rolls alongside the esophagus into the chest, with the junction remaining in the abdomen).
Sliding hiatal hernias are very common and are primarily associated with gastroesophageal reflux disease (GERD). Most are managed effectively with lifestyle modifications and proton pump inhibitor (PPI) medications. Surgery is indicated for patients with severe, refractory GERD unresponsive to medical therapy, large symptomatic hiatal hernias, complications such as esophageal stricture or Barrett's esophagus, or paraesophageal hernias (which carry a risk of gastric volvulus and strangulation and generally warrant elective repair).
The standard surgical procedure is laparoscopic fundoplication — typically Nissen fundoplication (360-degree wrap) or Toupet fundoplication (270-degree posterior wrap). The procedure involves reducing the herniated stomach back into the abdomen, narrowing the hiatus with crural sutures to prevent recurrence, and wrapping the fundus of the stomach around the lower esophagus to reinforce the lower esophageal sphincter and prevent reflux. A mesh may be placed at the hiatus for large defects to reduce recurrence.
