Hepatic Resection Or Lobectomy
Hepatic resection, including partial liver resection and hepatic lobectomy, is a major surgical procedure performed by expert hepatobiliary and surgical oncology surgeons at Moolchand Hospital — one of the best cancer and liver surgery hospitals in Delhi. It involves the surgical removal of a portion of the liver, ranging from a limited wedge resection of a small segment to an extended right or left hepatectomy removing up to 75–80% of the liver parenchyma.
The liver's remarkable regenerative capacity — it can regenerate to near-normal size within 6–8 weeks following resection — makes major hepatic resection possible, provided sufficient functional liver remnant is preserved.
Hepatic resection is indicated for primary liver tumours including hepatocellular carcinoma (HCC) arising in cirrhotic or non-cirrhotic livers, cholangiocarcinoma (bile duct cancer), and gallbladder cancer with liver invasion. It is also the treatment of choice for colorectal cancer liver metastases, neuroendocrine tumour liver metastases, and selected non-colorectal liver metastases. Benign indications include large haemangiomas causing symptoms, hepatic adenomas (particularly in women on oral contraceptives), and complex intrahepatic bile duct stones.
Preoperative assessment is crucial and includes liver function tests, volumetric CT assessment of future liver remnant, and in cirrhotic patients, assessment of portal hypertension and Child-Pugh or MELD score. Portal vein embolization (PVE) may be performed preoperatively to induce hypertrophy of the future remnant liver when it is considered too small for safe resection.
Intraoperative techniques include ultrasound-guided resection, vascular control with Pringle's manoeuvre, and various parenchymal transection methods. Minimally invasive laparoscopic or robotic hepatic resection is increasingly performed at Moolchand Hospital for selected cases, offering reduced blood loss, shorter hospital stay, and faster recovery.
