Thrombolytic Therapy
Thrombolysis — the pharmacological dissolution of acute blood clots using thrombolytic agents — is a critical emergency and interventional therapy performed at Moolchand Hospital, one of the best cardiovascular and neurology hospitals in Delhi. It is one of the most time-sensitive medical treatments available, where every minute of delay significantly worsens outcomes by allowing progressive, potentially irreversible ischaemic tissue damage.
Thrombolytic agents — including alteplase (rt-PA), tenecteplase, streptokinase, and urokinase — work by activating plasminogen to plasmin, which directly dissolves fibrin clots. Systemic thrombolysis involves intravenous administration and is used for acute ischaemic stroke (within 4.5 hours of symptom onset), massive or submassive pulmonary embolism (PE) with haemodynamic compromise, and in selected cases of acute STEMI where primary PCI is not available within acceptable time windows. Catheter-directed thrombolysis (CDT) delivers the thrombolytic agent directly into the clot through an intra-arterial catheter, used for acute peripheral arterial occlusion, ileofemoral deep vein thrombosis (DVT), and selected pulmonary embolism cases requiring more controlled delivery.
At Moolchand Hospital in Lajpat Nagar, the decision to administer thrombolysis requires rapid clinical assessment of eligibility (strict inclusion and exclusion criteria to minimise bleeding risk), imaging (CT/MRI for stroke, CT pulmonary angiography for PE, angiography for peripheral occlusion), and informed consent. A dedicated stroke team and cardiac emergency team are available 24 hours at Moolchand Hospital to evaluate and treat eligible patients within the critical treatment windows.
For acute ischaemic stroke, Moolchand Hospital follows the national stroke protocol — CT brain to exclude haemorrhage, IV alteplase within 4.5 hours of last known well time, and close neurological monitoring for 24 hours post-thrombolysis in our stroke unit. For massive PE with cardiac arrest or haemodynamic collapse, immediate systemic thrombolysis is administered even in the absence of imaging confirmation. For acute STEMI without access to timely primary PCI, thrombolysis with tenecteplase is a life-saving alternative.
