Temporary Cardiac Pacing
Temporary cardiac pacing is a critical, potentially life-saving procedure performed at Moolchand Hospital, one of the best cardiac hospitals in Delhi. It involves delivering electrical impulses to the heart via a pacing electrode catheter to maintain an adequate heart rate in patients with dangerously slow heart rhythms (bradycardia) or complete loss of normal electrical conduction that have caused haemodynamic compromise.
Life-threatening bradycardia may arise from complete heart block (third-degree AV block — where no electrical impulses pass from the atria to ventricles), high-grade second-degree AV block, symptomatic sinus bradycardia or sinus arrest, bradycardia following acute myocardial infarction, drug toxicity (beta-blockers, digoxin, calcium channel blockers), electrolyte disturbances (severe hyperkalaemia), or post-cardiac surgery conduction abnormalities.
At Moolchand Hospital in Lajpat Nagar, temporary transvenous pacing is the most reliable and effective method for emergency pacing. Under local anaesthesia and using fluoroscopic or echocardiographic guidance, a bipolar pacing electrode catheter is advanced through a central vein (right femoral, right internal jugular, or subclavian) into the right ventricular apex, where it is positioned to achieve stable sensing and pacing capture. The catheter is connected to an external pacing generator that delivers precisely calibrated electrical impulses at a set rate and output.
Transcutaneous pacing — using adhesive electrode pads on the chest wall connected to an external defibrillator with pacing capability — is used as an immediate bridging measure for emergency bradycardia while transvenous access is established. It is painful in conscious patients and less reliable than transvenous pacing.
Post-insertion, pacing thresholds are verified and the pacing rate and output are optimised. The temporary pacemaker is maintained until the underlying cause is treated (e.g., resolution of drug toxicity, recovery from myocardial infarction) or until a permanent pacemaker is implanted by Moolchand Hospital's cardiac electrophysiology team.
