Implantable Cardioverter Defibrillator (ICD) and combination devices are advanced cardiac implant procedures performed by expert cardiac electrophysiologists at Moolchand Hospital, the best heart hospital in Delhi. These devices represent the gold standard for prevention of sudden cardiac death (SCD) from life-threatening ventricular arrhythmias — ventricular tachycardia (VT) and ventricular fibrillation (VF) — which cause the majority of sudden cardiac deaths worldwide.
An ICD is a small, battery-powered device implanted beneath the skin below the collarbone, connected to one or more leads (insulated wires) threaded through a vein into the heart chambers. The device continuously monitors the heart's electrical activity. When a life-threatening arrhythmia is detected, it delivers a precisely calibrated electric shock (defibrillation) to restore normal rhythm — all within seconds, without any action required from the patient.
Modern ICDs also function as pacemakers, delivering anti-tachycardia pacing (ATP) — a series of rapid pacing pulses that can painlessly terminate many ventricular tachycardias without a shock — and backup bradycardia pacing when the heart rate is too slow.
ICD implantation is indicated for survivors of sudden cardiac arrest, patients with sustained VT causing haemodynamic compromise, those with severely reduced left ventricular function (ejection fraction ≤35%) due to ischaemic or non-ischaemic cardiomyopathy at high risk of sudden death, and patients with inherited arrhythmia syndromes such as hypertrophic cardiomyopathy, long QT syndrome, Brugada syndrome, and arrhythmogenic right ventricular cardiomyopathy (ARVC).
Combo devices — CRT-D (Cardiac Resynchronization Therapy Defibrillator) — combine ICD protection with cardiac resynchronization therapy (CRT) for patients with heart failure, reduced ejection fraction, and bundle branch block causing dyssynchronous ventricular contraction. CRT-D delivers coordinated pacing to both ventricles simultaneously, improving cardiac efficiency, symptoms, exercise capacity, and survival in appropriate heart failure patients while providing ICD protection.
