Partial Hip Replacement (Hemiarthroplasty) Partial hip replacement, or hemiarthroplasty, involves replacing only the femoral head (ball) component of the hip joint while leaving the natural acetabulum (socket) intact. It is most commonly performed for displaced femoral neck fractures in elderly patients — particularly following a fall — where preserving the acetabulum is appropriate as the socket cartilage remains healthy. The procedure is faster, involves less blood loss, and is associated with lower surgical risk compared to total hip replacement, making it well suited for frail elderly patients. Implant options include unipolar (fixed head) and bipolar (mobile head) hemiarthroplasty, with the bipolar design offering slightly better functional outcomes.
Total Hip Replacement (Total Hip Arthroplasty — THA) Total hip replacement is the gold standard surgical treatment for end-stage hip arthritis and is the most common type of hip replacement performed at Moolchand Hospital. The procedure involves replacing both the femoral head (ball) and the acetabulum (socket) with prosthetic components. The acetabular cup is implanted into the pelvis, a liner (polyethylene, ceramic, or metal) is placed inside the cup, and a femoral stem is inserted into the femoral canal topped with an artificial femoral head. Modern bearing combinations include ceramic-on-ceramic, ceramic-on-polyethylene, and metal-on-polyethylene, each with specific advantages in terms of wear resistance and longevity.

Revision surgery is significantly more challenging than primary replacement due to bone loss, scarring from previous surgery, and the need to remove well-fixed implants.
Revision procedures may involve partial revision (replacing only one component — femoral stem or acetabular cup) or full revision (replacing all components simultaneously). Bone grafting — using autograft, allograft, or synthetic bone substitutes — is often required to restore bone stock lost due to osteolysis around the failed implant. Highly porous revision implants with ingrowth surfaces provide stable biological fixation in deficient bone.
